USA Boxing Registration

 

 

Every Boxer must comptete the following steps to register with USA Boxing.

 

USA Boxing Checklist

 

1. USA Boxing Medical Certificate Form.We provide it for you to be filled out by your doctor.

 

2. Copy of your birth certificate 

 

3. Two passport size photos (Walmart, CVS, Walgreens)

 

4. $60 membership fee to be completed on line at; USAboxing.org (you must list StickNMove as your club affiliation)

 

5. Print out Membership Card

 

6.  Physical (have you doctor complete the form)

 

7. Hand in documentation to Stick N Move

 

8. Your book will be mailed to your home.

 

9. Give your book to Coach Antwoine
